In the process of taking down the Christmas lights, Rachel injures her ankle. She and Monica go to the hospital but Rachel doesn’t have any health insurance. Monica switches names with Rachel so that Rachel can use Monica’s insurance. By good fortune, they meet two handsome doctors. Later the doctors show up at Monica’s apartment for a date. Rachel thinks they should tell the doctors their real names, but Monica insists that they shouldn’t because they might get into trouble for insurance fraud. This leads to an argument between the two and it messes up their date. After this, they decide to just pay the doctor bill with their own money and forget all the trouble that the insurance switch has caused.

The gang have planned a surprise birthday party for Phoebe at Monica’s apartment. When they hear a knock at the door they think Phoebe has arrived. Monica yanks open the door and everyone yells “surprise!” Ross, who was the one at the door, is so startled that he drops Phoebe’s birthday cake and it lands upside down on the floor.

Joey continues to date Ursula and neglects Phoebe to the point of missing her birthday party. In the meantime, Ursula has stopped returning Joey’s phone calls. Phoebe feels bad for Joey and goes to talk to Ursula about it. Ursula tells Phoebe it's over between her and Joey, and she gives Phoebe the dress that Joey had given her as a birthday gift. Phoebe decides to wear the dress and pretend to be Ursula. She meets with Joey at the coffeehouse. Phoebe, as Ursula, insists that Joey must choose between her and Phoebe. Joey chooses Phoebe so they break up. They have a “good-bye” kiss. After the kiss Joey realizes that Ursula is really Phoebe in disguise. He is touched that she tried to keep him from heartbreak while protecting their friendship.

Ross discusses his fears of being a bad father with his own father. He tells Joey and Chandler about a dream where he throws his son like a football and then realizes he has to be the one to run down field and catch him. The guys try to reassure Ross that he’ll make a good father. Later, during a game of Scrabble, Marcel begins gagging on Scrabble tiles that he swallowed. Ross takes Marcel to the hospital. At the end Chandler assures Ross that what he just did for Marcel proves that he’ll be a good father.
